We live and work in the community. We are committed to providing access to quality health care services to our Participants. In addition, AmeriHealth Caritas Pennsylvania (PA) Community HealthChoices (CHC) connects with the communities we serve. Through participation in community outreach events, health fairs, and neighborhood functions, we make a real difference in our Participants' lives. We lend support to, and work with, Participants and community groups to foster Participant independence by addressing issues that affect health care, including housing, transportation, community transition services, employment skills, and health education.
Together, we can support healthy living for all.
Tools for Tomorrow
Tools for Tomorrow is AmeriHealth Caritas PA CHC's job readiness series, featuring industry leaders who will give practical advice for landing the ideal job. All AmeriHealth Caritas PA CHC Participants are invited to attend.
Community Health Worker listening session
The Community Health Worker ecosystem is a critical component of Public Health, enhancing access to services, addressing social determinants and root causes of health issues, and improving the quality and cultural competency of health service delivery. As a Community Health Worker, your role plays an integral part in the success of this ecosystem.
Please consider joining the Pennsylvania Department of Human Services (DHS) Office of Long-Term Living (OLTL) for a workshop-format listening session where you will be able to engage with other Community Health Workers and discuss topics including challenges faced, successes achieved, and relationship and coordination strategies with other stakeholder groups, and provide perspectives on other key topics. This listening session will be offered virtually.
