As an AmeriHealth Caritas Pennsylvania (PA) Community HealthChoices (CHC) Participant, you have a network of committed health care providers ready to take care of you. Your providers are your partners in care.

Use the tools below to find health care providers in the AmeriHealth Caritas PA CHC network.

Find the care you need

• Dual-eligible Participants: Please refer to the drug list of your Medicare Part D plan.
• If you have AmeriHealth Caritas PA CHC as a secondary coverage, use our list of covered over-the-counter medicines. You can also view the printable version (PDF) here.
• If you have AmeriHealth Caritas VIP Care, use our Medicare Part D formulary.
• Medicaid LTSS Participants:

  On January 1, 2020, the Pennsylvania Department of Human Services (DHS) made changes in the way that some medicines are covered. All Pennsylvania Medical Assistance (Medicaid) health plans were required to move to a statewide preferred drug list (PDL) (PDF). If you have any questions about this change, please call Participant Services at 1-855-235-5115 (TTY 1-855-235-5112).
